CASE MANAGEMENT A comprehensive assessment of a client's needs is determined via an intake interview. A Care Plan is developed based on those needs, and a client's progress is monitored monthly. A bi-annual reassessment modified Care Plan if needed.
HOMEMAKER In-home assistance is arranged for the client who needs help with meal preparation, basic household tasks and light cleaning.
PERSONAL CARE In-home assistance is arranged for frail clients who are unable to perform at least two activities of daily living such as bathing, dressing and/or feeding. Help is also arranged if a client's personal safety is in question.
IN-HOME RESPITE Temporary assistance is provided for a client needing a caregiver when their regular caregiver is resting from the duties of caring for the client.
OUTREACH Case Management staff identifies potential clients and encourages the community to take advantage of existing services. This is done by group or one-to-one presentations.
REGISTRY Case Management staff compiles and maintains a list of service providers offering services for hire. A background check and screening is conducted of all potential service providers prior to being placed on the Registry List.

REASSURANCE
Case Management volunteers make weekly telephone calls to homebound seniors to provide social contact, and to check on the safety and well-being of the clients.
NUTRITION The Joslyn Adult Center meets seniors' nutritional needs by providing services such as a congregate meals program (held Monday-Friday at 11 a.m.). The Center also offers a Home-Delivered Meals program and Food Bank referrals.
TRANSPORTATION Clients unable to drive can register for Senior Ride - a free curb-to-curb transportation service within City limits.
HOUSING Case Management staff provide referrals and assistance placing clients in senior apartments, shared housing, retirement communities or group homes.
EQUIPMENT Assistive equipment, such as walkers, commodes, wheelchairs, etc., are arranged for delivery through the Convalescent Aid Society.
GENERAL ASSISTANCE The Case Management Project focuses on the well-being of the senior community by providing assistance with Social Security issues, senior advocacy, landlord/tenant issues, consultations and more.
