Case Management Assists Functionally-Impaired Adults

For almost 20 years, the City of Alhambra has provided Case Management Services to functionally-impaired, Alhambra-resident, adults (physically, emotionally, or mentally-challenged), 18 years of age and older. The variety of services available from the Case Management Project office at Joslyn Adult Center, 210 North Chapel Avenue, are designed to assist individuals in remaining independent and self-fulfilled for as long as possible, and include referrals, advocacy, service and Caregiver resources, provision of in-home medical equipment, and more.

If you know someone who could benefit from these services, please refer to them to the Case Management office. Call (626) 570-3220, during weekday business hours, for an appointment with one of the project Social Workers (who speak English, Spanish and Mandarin). You can discuss any personal concerns, and find out if the project services can enhance your personal situation.

If you have health care questions, or need assistance with finding out what your Medi-Cal or Medicare benefits are, the Case Management Project can help. They can also assist you with mental health and medical referrals.

If you live in your home and need assistance with in-home medical supplies, they can assist you with obtaining equipment, such as wheelchairs, walkers, canes, grab bars, and much more. They can also assist you with In-Home service referrals and Caregiver resources.

The Case Management Project can provide assistance and advocacy with your Social Security issues, and refer you to an Elder law Attorney, for free legal consultations in matters of wills, trusts, probate and other estate planning matters. They also provide referrals for housing in the area if you are unable to remain in your current residence.

Case Management Project Social Workers can also assist you with other City of Alhambra services, such as Senior Ride (free transportation service for disabled residents) and the variety of leisure activities available at Joslyn Center (including leisure classes, adult education classes, special events and outings).
